Informed Consent for Telehealth Services

Introduction

This document explains telehealth services offered through GetPepWell, Inc. ("GetPepWell"). By using our telehealth services, you acknowledge that you have read, understood, and voluntarily consent to receive healthcare services via telehealth technology. Please read this consent carefully before scheduling a consultation.

Telehealth is the delivery of healthcare services using electronic communications and information technology when the provider and patient are not in the same physical location. Telehealth at GetPepWell includes assessment, clinical decision-making, and prescribing of compounded peptide preparations through our secure platform.

Nature of Telehealth Services at GetPepWell

Your telehealth visit at GetPepWell consists of a real-time video and audio consultation with a physician who is licensed in the state where you are physically located at the time of the visit. Your medical history, the eligibility-screening responses you provide, and any prior records you share are reviewed by the physician before and during the visit.

The types of telehealth services offered through GetPepWell include:

  • Initial patient consultations to evaluate eligibility for peptide therapy
  • Follow-up visits to monitor treatment progress and adjust protocols
  • Prescription management and medication review
  • Medical-intake review and clinical decision-making
  • Patient education regarding peptide therapies and treatment safety

Telehealth is not a replacement for in-person care in every situation. Your physician will determine whether telehealth is appropriate for your specific clinical needs and may recommend in-person care when warranted.

Compounded Medications and Provider Discretion

GetPepWell does not manufacture medications. Compounded medications obtained through GetPepWell are prepared by licensed compounding pharmacies operating under applicable state and federal pharmacy regulations. Compounded medications are not interchangeable with the branded medications they may reference, and actual product appearance and labeling may differ from any website images.

The GetPepWell eligibility assessment and your telehealth consultation are the basis on which a physician determines eligibility for treatment. Providers retain full discretion to prescribe or decline to prescribe any medication based on their professional medical judgment. Results vary by patient and depend on adherence, provider guidance, and lifestyle factors. Patient experiences referenced elsewhere on the website reflect individual outcomes and are not guarantees of similar results.

  • Reduced travel: no need to travel to a clinic
  • Timely access: often faster scheduling than in-person care

Risks and Limitations

As with any healthcare service, telehealth has limitations and risks. You should consider the following before consenting:

Technology limitations

  • Technical failures (internet outage, audio or video interruptions) may delay or disrupt a consultation
  • Electronic systems and safeguards may fail. Although we apply HIPAA-compliant safeguards, no electronic system is risk-free
  • Information shared during a consultation may not be sufficient for the physician to reach a clinical conclusion, in which case the physician may recommend additional evaluation or in-person care

Clinical limitations

  • The absence of a hands-on physical examination may limit the physician's ability to diagnose certain conditions
  • Delays in evaluation and treatment may occur due to scheduling, pharmacy workload, or technology
  • In some cases, the physician may determine that telehealth is not appropriate for your needs and recommend an in-person visit with a local provider

Privacy risks

  • Despite reasonable security measures, there is always some risk that electronic communications could be intercepted
  • You are responsible for ensuring that your environment during a telehealth consultation is private and free from unauthorized listeners

Your Rights

As a patient receiving telehealth services through GetPepWell you have the right to:

  • Withdraw consent: withdraw your consent to receive telehealth services at any time, verbally or in writing, without affecting your right to other care
  • Request in-person care: request an in-person consultation. Your physician may also recommend in-person care when clinically appropriate
  • Access your records: access the medical records generated during telehealth consultations, subject to applicable law (see our Privacy Policy and Notice of Privacy Practices)
  • Know your provider: know the identity, state license, and qualifications of the physician providing your telehealth consultation
  • Privacy: have your consultation conducted in a private and secure manner in compliance with HIPAA regulations

Provider Qualifications and State Licensure

Each physician on the GetPepWell platform is licensed in the state where you are physically located at the time of your consultation. Before scheduling a visit, you will be asked to confirm your current state of residence and your physical location at the time of the visit. If you relocate or travel outside your home state, please inform us so we can match you with a physician licensed in your current state - the rules of telemedicine licensure depend on your physical location during the call, not your home address.

Technical Requirements

  • A stable internet connection (broadband recommended, minimum 1.5 Mbps upload/download)
  • A computer, tablet, or smartphone with a current web browser (Chrome, Firefox, Safari, or Edge)
  • A camera and microphone for video consultations
  • A private, quiet location where you can speak freely about your health
  • A valid email address for confirmations, notifications, and follow-up

Privacy and Security

GetPepWell is committed to protecting the privacy and security of your health information during telehealth consultations:

  • Telehealth sessions are conducted over HIPAA-compliant, encrypted communication channels
  • We apply technical safeguards including encryption in transit and at rest, access controls, and audit logging
  • Telehealth consultations are not recorded by GetPepWell unless you are specifically informed and provide separate consent
  • Your physician may take clinical notes during the consultation; those notes become part of your medical record
  • All GetPepWell personnel with access to your health information are trained on HIPAA compliance and bound by confidentiality obligations

Prescriptions via Telehealth

Physicians on the GetPepWell platform may prescribe medications, including compounded peptide preparations, based on the findings of your telehealth consultation. You understand and agree that:

  • All prescribing decisions are at the sole discretion of the treating physician
  • A telehealth consultation does not guarantee that any prescription will be issued
  • Prescriptions are transmitted electronically to a licensed compounding pharmacy for fulfillment
  • You are responsible for following the prescribed dosage, administration instructions, and any other safety guidance provided directly by your physician
  • Certain medications may be subject to state-specific rules and restrictions
  • New York State requires electronic prescribing for all prescriptions under the I-STOP program

Emergency Situations

Telehealth is not intended for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately.

Examples of medical emergencies include:

  • Difficulty breathing or shortness of breath
  • Chest pain or pressure
  • Severe allergic reactions (anaphylaxis)
  • Loss of consciousness or seizures
  • Severe or uncontrolled bleeding
  • Sudden numbness, confusion, or difficulty speaking
  • Thoughts of self-harm or harm to others

If a medical emergency occurs during a telehealth consultation, your physician will advise you to call 911 and may help direct emergency services to your location if possible.

State-Specific Requirements

Telehealth regulations vary by state. GetPepWell operates in compliance with the telehealth laws and regulations of each state in which we provide services. Some state-specific considerations:

  • Physicians providing telehealth consultations are licensed in the state where you are physically located at the time of the consultation
  • Certain states (California, Florida, Kentucky, Louisiana, Mississippi, Texas, and others) require explicit telehealth informed-consent acknowledgment. Where state law imposes additional disclosure or consent steps, you will be presented with those steps before your visit
  • Some states impose limitations on the types of medications that may be prescribed via telehealth
  • You must be physically located within the United States during your telehealth consultation

Acknowledgment

By proceeding with a telehealth consultation through GetPepWell, you affirm that:

  1. You have read and understand this Informed Consent for Telehealth Services
  2. You understand the potential benefits, risks, and limitations of telehealth
  3. You voluntarily consent to receive healthcare services through telehealth
  4. You understand that you may withdraw your consent at any time without penalty
  5. You have had the opportunity to ask questions about telehealth services, and any questions you had have been answered to your satisfaction
  6. You understand that telehealth is not appropriate for medical emergencies and that you should call 911 in case of an emergency
  7. You understand that providers retain full discretion to prescribe or decline to prescribe any medication, and that compounded medications are prepared by licensed compounding pharmacies and are not interchangeable with the branded medications they may reference

Your acknowledgment is captured electronically when you complete the pre-consultation consent step in the patient portal. The platform records the date, time, and a record of the version of this notice you affirmed.
